PRIVACY NOTICE — Brain Death Determination Certification

Who holds your data. The Philippine Neurological Association (PNA) is the data
controller. The platform operator processes this data on PNA's instructions.

What is collected. Only your full name as it is to appear on your certificate,
your PRC license number, your email address, and a password. In the course of
the program the platform also records your assessment attempts, your workshop
attendance, and your certificate.

What is NOT collected. No photograph, no selfie, no image of your PRC
identification card, and no biometric data of any kind. Your identity is
confirmed in person, by PNA staff inspecting your physical PRC card at the
simulation workshop.

About your PRC license number. It is recorded as you enter it and is not checked
against the Professional Regulation Commission or any other authority. It is
used to identify your certificate on the public register, and staff may correct
it at workshop check-in if it does not match your card.

Why it is collected. To deliver the program, to record your assessments and
attendance, to issue and verify your certificate, and to report on program
outcomes in aggregate.

What is published. If you are certified, your name, certificate number, validity
dates and status appear on a public register, searchable by PRC license number
or certificate number. This is how a hospital confirms a certification is real.

Retention. Assessment and certificate records are kept for the life of the
certificate and a period afterwards. PNA has not yet fixed those periods; this
notice will be updated and re-issued when it does.

Your rights. You may ask for access to your data, correction of it, or its
erasure, by contacting PNA. Erasure is limited where a certificate has been
issued: a public register that can be selectively emptied cannot be relied upon,
so an issued certificate record is retained even where other data is removed.
